Vital Statistics Applications and Forms

Non-Genealogical NJ Vitals Application include:
Spanish- Aplicacion para copias certificadas o certificaciones de registros civiles no-ancestro

• Birth Certificate records within the last 80 years.
• Marriage Certificate records within the last 50 years.
• Death Certificate records within the last 40 years.

Genealogical NJ Vitals Application include:
Spanish- Aplicacion por una copia certificada o certificaciones de un registro civil ancestro

• Birth Certificate records older than 80 years.
• Marriage Certificate records older than 50 years.
• Death Certificate records older than 40 yeas.

Marriage, Remarriage, or Civil Union License Application
